Invention Disclosure Form - Use this form when disclosing an invention to the University

Statement of Financial Interest - Use this form when an inventor has a relationship with an outside entity that needs to be disclosed to the University.

Confidentiality Agreements - Use these form when discussing proprietary information to anyone outside of the University

        Mutual Non-Disclosure Agreement (Both Parties Disclosing Information)

Material Transfer Agreement - Use this form when tranfserring biological or chemical materials OUTSIDE of the University. Please contact Office of Technology Transfer at ext. 7007 when transferring material INTO the University.
